The Transition Improvement Grant has designed a NEW transition application (app). The WiTransition App is a free, web-based tool developed by the Transition Improvement Grant (TIG) to support meaningful student participation in transition planning. It is designed to center the student’s voice by creating space for students’ to reflect on their transition goals and future plans in a structured and supportive way.

The app emphasizes self-reflection, communication, and shared understanding. Students use the app to express what matters most to them as they think about life after high school, including education and training, employment, and independent living. This information can then be brought into IEP Team conversations to help inform the development of the student’s Postsecondary Transition Plan (PTP). The WiTransition App report does not take the place of the PTP.
The app also supports collaboration between students, families, educators, and transition partners by creating a common starting point for transition discussions. When everyone can see the student’s perspectives and priorities, planning becomes more intentional, personalized, and aligned with the student’s long-term goals.
While designed for use in Wisconsin and aligned with state and federal transition requirements under IDEA, the WiTransition App may also be used in other states as a general transition planning support.

WiTransition App Paper Application:The paper application is intended to be used when technology isn’t readily available or for youth to complete ahead of time and input information in one sitting. The application does not save, but a pdf can be generated and emailed to individuals the student chooses.
